In accordance with the condition of the Wyoming Department of Environmental Quality/Water Quality
Division Permit to Construct No. 2020 - 12 , requiring submittal of this Certification of Completion
within sixty (60) days of completion of the facility, I hereby certify:
Construction of the permitted facility was completed on

submitted in support of the application.

provi-sions of Section 11, Chapter 3, Wyoming Water Quality Rules and Regulations.
As -built plans and specifications, certified by a registered professional engineer
(certification by an engineer is not required if the original application was not certified
by an engineer) are enclosed.
